Billed per service at time-of-service rate. CC or ACH required — no exceptions. All services Mon–Sun invoiced the following Monday.

Pre-Funded
FFS · No Weekly Invoice
Pre-fund your account; services drawn as rendered. Min. $2,500 deposit. 20% replenishment trigger. CC or ACH recommended.
FLAT FEE — FFPE

All Covered Services Included
Type 2 accounts only. Min. $10,000 deposit. Ancillary services billed separately at rates above. Max 2 MRI/CT scans per employee per year.

Enterprise
Benefits programs require a separate Benefits Service Agreement and Health Services Account Authorization. PEPM covers MSK, virtual urgent care, and personal wellness — non-work-related conditions only.
MRI / CT Limit: Max 2 MRI/CT/Arthrogram scans per eligible employee per contract year — all account types.
Flat Fee: $950/episode covers all primary services for the episode. Ancillaries billed separately at rates above.
Rate Lock: FFS billed at time-of-service rate. Pre-funded balances serve as a de facto rate lock until depleted.
